Appointment Policy:

Our patients are committed, respectful individuals that value our time and expect the same from us!
We seek to minimize patient wait time, reserve treatment specific time intervals, and maximize efficiency for all.
All appointments made in our office are attempted to be confirmed by email, phone, or text messages well in advance of a scheduled visit.
It is critical we have updated contact information to effectively communicate with you.
Please kindly confirm your appointment with us in return!

Broken Appointment Policy:

In the event you must cancel your appointment please provide us two business days advance notice. This allows us sufficient time to schedule another patient awaiting treatment.
Thank you for your consideration for our time!

In the event we have not received your return confirmation one business day in advance of treatment we will assume the appointment was forgotten and we will cancel the time scheduled with us and offer this time to an awaiting patient.
A message to you will be left by email, phone, and text message to contact us to reschedule when you can commit to an appointment time.

SAME DAY CANCELLATIONS or MESSAGES LEFT AFTER BUSINESS HOURS or WEEKENDS to Cancel an Appointment has an adverse effect on our our business.
Everyone(including us!) has situations arise which are beyond their control.
Please realize short notice cancellations leave us with the inability to fill our time.

We seek to minimize REPEATED cancellations from the SAME INDIVIDUALS.
Following the SECOND SAME DAY Canceled Appointment the patient will receive a letter reminding them of our cancellation policy. A $150 Rescheduling Fee will be charged to schedule the THIRD appointment for the same procedure.
In the unlikely event we are unable to convince a given patient to otherwise alter their behavior we reserve the right to terminate our relationship and provide a referral to an alternative practice.

Financial Policy:

We are concerned about your dental health and strive to make dental care financially possible for each and every patient! We have a long practice history of assisting our valued patients make their dental goals a reality! We offer the following guidelines for payment of services:

Prepayment:
5% courtesy to any patient that pays in full at the time their appointment is made!

Time of Service:
We accept 50% down; 50% at case completion by: Check, Cash, Mastercard, Visa, Discover, or American Express

Payment Plans:
We currently are proud to offer payment plans through a popular company known as CARE CREDIT. This company offers financing solely for dental treatment. We have found this to be a great option for patients that present with many dental concerns and large treatment plans. No prepayment fees and low interest rates are possible via a simple, confidential application.
This plan has worked well for a number of our patients! Call us to see if this method of payment could work for you!

Dental Insurance:
If you are covered by an indemnity type plan(you are free to choose your own dentist!)
or by Delta Dental (we are Premiere Providers!) we will file your claim from our office on your behalf at the time of service.
*Please Note: Dental Insurance will only cover a portion of the total treatment costs.
Any remaining balance not received from your insurance company becomes the patient’s financial responsibility. We will work on your behalf to assure you receive every benefit dollar possible from your insurance company!
